Consent Statement

regarding the processing of personal data

 

I have been informed about the provisions of Regulation (EU) 2016/679 of 27 April 2016 (General Data Protection Regulation – GDPR) concerning the protection of individuals with regard to the processing of personal data and the free movement of such data, which repeals Directive 95/46/EC.

  • The right to intervene in the data;

  • The right not to be subject to automated individual decision-making;

  • The right to file a complaint with the supervisory authority.

I have been informed that my personal data, and the data of my minor child/children or spouse, may be collected and processed solely for legal purposes by the Medical Clinic CMI Dr. Lawrence Chukwudi Nwabudike, in compliance with applicable legislation.

 

The processing of this data is necessary for the clinic’s legal obligations and for exercising my rights and legitimate interests. The data will be stored only for the purposes stated above.

 

I have been informed that my personal data may be disclosed to authorized public institutions, such as: ANAF, ANFP, ITM, ANI, courts of law, criminal investigation bodies, or other legal authorities when required by law.

 

I understand that I am responsible for informing the clinic of any changes to my personal data to ensure accurate processing.

 

I also understand that I have the right to withdraw my consent at any time, by submitting a written, dated, and signed request to the Medical Clinic CMI Dr. Lawrence Chukwudi Nwabudike.
